It’s time for the Flu and COVID shot clinic again!
What you need to know for the COVID shot…
This opportunity is only available if the Agency can get a minimum of 80 people registered.
In order to guarantee a shot please complete the registration form below. If you’re registering for a spouse or family member please register separately for each person (one registration = one shot). This will help us easily get an accurate count. You may use your email more than once, so that we may contact you if there are any questions, but please use individual names.
*This year, the COVID Vaccine is the Pfizer brand.
Please bring your insurance card and note that there will be a fee for No Shows.
